Southcoast Visiting Nurse Association to offer free health assessments, screenings in Rochester Aug. 16
The Southcoast Visiting Nurse Association will be offering free health assessments, screenings and wellness education for seniors in Rochester on Thursday, August 16 from 10 a.m. to 11:30 a.m. at the Council on Aging located at 67 Dexter Lane.
This screening will include a free medication review with a pharmacist. Interested participants are invited to bring their medications and supplements to this screening for a review that will include identification of medication errors, review of medication instructions and advice on how to best take individual medications.
The program also provides individual, in-home assessments for those who cannot attend a community clinic. The individual assessment with a Southcoast VNA nurse includes a personal health assessment, an assessment of the individual’s medications as well as one on one wellness education.
Participants identified as having a need for better management of a chronic disease may be offered an in-home monitor that will take daily vital signs in the comfort of the individual’s home. A Southcoast VNA telemonitor nurse will monitor the vitals and communicate trends with the participant’s physician.
The program will also provide free immunization education and immunization clinics to improve the immunization rate of the elderly in the Greater New Bedford region.
